Hospital Discharge
A trip to the hospital can be an intimidating event for patients and their families. As a caregiver, you are focused completely on your family member or friend’s medical care, and so is the hospital staff. You might not be giving much thought to what will happen when your friend or family member leaves the hospital.
This will include:
1.Final examination of the patient;
2.Discussion of the hospital stay, even if the time spent by the physician on that date is not continuous;
3.Instructions for continuing care to all relevant caregivers; and
4.Preparation of discharge records, prescriptions, and referral forms
Or, you may transition to home care. For this, we will work with your healthcare providers to manage your care at home. A home care agency may send healthcare providers to your home to check in with your progress.
